ADMISSIONS: Information & Tour
It is hard to describe our facility and express our true feeling of family you will have when you become part of our OIH. However, if you would like try and get a sense of this, please come for a visit and/or tour of our facility. You will truly find it a wonderful, unique experience.
We are happy to meet with families and potential residents to answer any questions or provide information at your convenience. The admission process requires information gather prior to date of admission including, but not limited to, medical and social history, advance directives, as well as financial assets and insurances. WHO ARE WE?

Our Department of Public Health Survey for 2010 resulted in Zero Deficiencies! We are very proud of all the hard work from every level of our staff that allowed this to be possible. A copy of our survey is posted on the bulletin board at OIH.
OUR ISLAND HOME is a 45-bed smoke-free Skilled Nursing Facility dually licensed in Medicaid and Medicare since August 2007 and owned by the Town of Nantucket. Our rooms are primarily semi-private; however, we do have three Private ‘Special Care’ rooms available. Each has a large window; some with a view of the creeks and Nantucket Harbor where we are located, as well as cable and telephone ready access. We have a unique tropical fish tank in the living room area, a beautiful out door patio overlooking the harbor with volunteer manicured gardens, and daily pet visits that are always a treat. Our Resident population is diverse in not only heritage but origin of location. You will find men and women whose family roots go deep into Nantucket soil and those who have come to join family after coming to the island
to visit and choosing to stay.

We strive to provide quality care based upon customized care plans to meet each individual resident’s needs while preserving their independence and dignity. Each plan of care is constructed to enhance each resident’s physical, emotional, social and mental well-being and is based upon individual abilities, goals, and preferences. We understand that the environment is just as important as the care a resident receives.
We are committed to providing a comfortable, home-like atmosphere and ensure our team of staff and all caregivers are only those who bring enthusiasm, dedication, compassion and warmth to their work. We are very proud of the large number of our staff that has been with us for many, many years.
Whether your stay is short term or for several years, we want it to be as positive of an experience as possible. All residents are strongly encouraged to bring in their own personal items that will lend to the home-like atmosphere to their rooms that we want to achieve.
Additional Services offered that are not included in the basic daily rates or under Medicaid:
Hairdresser/Stylist: OIH has a beauty parlor to meet all your wash, cut, blow dry, perm and coloring needs!
Church Services: we have ministers of multiple faiths regularly come and hold services including communion.
Cable Television: basic cable access is provided, however, premium channels are to be set up and paid for by the resident directly with the cable company.
Telephone: access for phone service and individual phone numbers is available, however, like cable the set up is paid for by the resident directly with the phone company. Cost depends on the type of service and telephone that is selected. We do encourage mobile and able residents to have cell phones for easier access in contacting loved ones and receiving phone calls.
Transportation: OIH’s van is available to provide on-island transportation to and from medical appointments for no charge. Off-island transportation, as needed, can also be arranged through the facility.
Laundry: laundry service is provided at no cost; however, there is currently no option for dry cleaning needs.
Newspaper: fees for newspaper delivery are based upon the particular newspapers current rates.
ACTIVITIES PROGRAM: Activity Director is Sybil Nickerson ( snickerson@nantucket-ma.gov ) OIH is very proud of its diverse Activities programming. It is planned to provide incentive and opportunity for the Residents to use mental, social and physical capabilities and skills. This includes games, movies, Art class, crafts, parties, discussion groups, homemaking activities, movement and exercise programming, gardening, field trips, clergy (ministers of all faiths are welcome and encourage to visit) and much more.
To list a few of the community activities our Residents participate in are: Daffodil Parade, Boston Pops Concert, Christmas Stroll and Tree Decorating, Whaling Museum trips, Annual OIH Fair, and the Coast Guard Valentine’s Day Ball.
REHABILITATION PROGRAM: Our rehabilitation program is offered 7 days a week. Therapy is offered under our Restorative Nursing Program or though skilled therapist (Physical, Occupational and Speech) that have expertise in both therapy evaluation and treatment. With a physician prescribed order; a treatment plan is developed that suits each individual’s needs. Our rehab services are designed to maintain each individual’s level of functioning, independence, or assist them in reaching their highest level of functioning as well as to assist in the transition from post acute care to home.
HOW TO HELP
VOLUNTEERING
OIH has a very strong and active volunteer program. Currently we have at least 25 members of the community who offer their time! We encourage anyone interested to bring their skills, talents or just time to Our Island Home by becoming a member of our volunteer team and OIH family.
CONTRIBUTIONS
Your donations help us continue to enrich the lives of our residents and further enhance our quality care provided. Often, individuals provide donations with specific activities or items in mind. We often find that many choose to donate to celebrate the lives of loved ones to sustain their memories. Whichever you choose, please know that every donation is greatly appreciated by all staff and residents.
OUR ISLAND HOME IS A DEPARTMENT OF THE TOWN OF NANTUCKET AND RECEIVES FUNDING FROM PRIVATE REVENUES, REIMBURSEMENT FROM MEDICAID, MEDICARE, AND SUBSIDY FROM THE TOWN.
