Town and County of Nantucket - ZBA Submission Requirements and Downloadable Forms

1.	Sixteen (16) separate application packets, which shall include a minimum of the following: • Complete application page • Addendum (if necessary) • Locus Plan from the Nantucket Tax Assessor’s Map Book • Surveyed plot plan of existing conditions • Surveyed plot plan of proposal (if applicable) • Abutters List (including the original stamped assessor’s copy) • If you are not the owner or applicant, a written statement of agency is required
2.	Two (2) separate sets of addressed mailing labels based on the abutter list provided by the Tax Assessor’s office. Mailing labels must be 1” x 2 5/8”
3.	A check for $300.00 made payable to the Town of Nantucket.
